IMPORTANT UPDATE:
We are sorry to hear that you have been a victim of electronic theft of your SNAP benefits. At this time, the agency is able to reimburse those benefits back to you, but you will need to complete the JFS 7011– Statement Requesting Replacement of Electronically Stolen Supplemental Nutrition Assistant Program (SNAP) Benefits. The JFS 7011 is to be filled out entirely, including your signature. The top page of the form provides more information on what skimming is and what the agency can do to replace those benefits. When completing the form, you will need your transaction history. You can access your transaction history by going online to www.connectebt.com. Once you have completed your JFS 7011 form, please return it to the agency either by ordinary mail, emailing pictures of the form to allen-fraud@jfs.ohio.gov, or dropping the form off to our main lobby. The agency then has 30 days to process your form once it has been received. The agency will need to verify that your benefits have been electronically stolen before we are able to reimburse them back. All forms are worked in date order, and you will be notified once it has been processed. You will also need to request a New EBT card, in order for the agency to reimburse your benefits. New EBT cards take 7-10 business days to arrive to you.

How to Report Fraud
If you suspect fraud in Medicaid, Food Assistance, Ohio Works First (including cash assistance and workforce development programs), or Child Care, please fill out this form or report it to (419) 228-2120. If you leave a voicemail, one of our investigators will follow up on your complaint.
